A Guide to Mental Health Treatment Plan Item Numbers for GPs and non-VR GPs

Overview

Eligibility

For a patient to be eligible for a mental health treatment plan, they must have a diagnosable mental health disorder

Plan Requirements

A mental health treatment plan must be documented in writing and needs to include an assessment and diagnosis of the patient's mental health condition, including administration of an outcome tool measure unless clinically inappropriate, as well as a plan for managing their illness. Examples of outcome tools could include a K10 or DASS21, but the choice of which is at the discretion of the practitioner.

The plan should include treatment options and goals, any patient education provided, a crisis intervention/prevention plan, as well as a plan for follow up or review.

The plan should then be saved to the patient file, a copy offered to the patient, and a copy sent to the treating psychologist, accompanied by a letter stating how may sessions the patient is able to access.

Billing

A mental health treatment plan can be billed no more often than once every 12 months, unless exceptional circumstance apply. A mental health treatment plan can be reviewed more regularly and can be billed no more than once in a 3-month period.

Sessions

A mental health treatment plan entitles a patient to access a Medicare rebate on up to 10 individual and 10 group sessions with a registered mental health professional per calendar year. The initial plan generally provides 6 sessions, and then further reviews can offer ongoing sessions as deemed appropriate, to a maximum of 10 per calendar year. These can roll over if not all used. A letter including the number of sessions granted must be included with the referral to an appropriate treating mental health professional

Review Requirements

A review must include comparison of the patients progress against the goals included in the original treatment plan. Education should be reinforced, a crisis plan confirmed, and an outcome tool should once again be used. The treatment plan should then be adjusted and modified as appropriate
